Waiver of Liability Kids Academy NY
Weekend School Program For Kindergarten through 5th Grade
This Waiver of Liability: (the "Waiver") is entered into by the undersigned parent/guardian on behalf of the child named in this form and Kids Academy NY as of the date signed below.
1. Acknowledgment of Risk : By signing this Waiver, the Parent/Guardian acknowledges and agrees that participation in the Kids Academy NY weekend school program, including but not limited to Arabic language classes, Quran memorization, soccer league activities, character education classes, and all other activities , may involve inherent risks, including the risk of injury, illness, or other unforeseeable events.
While the Academy takes reasonable precautions to ensure the safety of all participants, the Parent/Guardian acknowledges that the Academy cannot eliminate all risks associated with the Program.
2. Waiver and Release of Liability : In consideration of the Participant being allowed to enroll and participate in the Program, the Parent/Guardian, on behalf of the Participant and their heirs, assigns, personal representatives, and estate, does hereby voluntarily release, waive, discharge, and hold harmless the Academy, its directors, officers, employees, agents, and volunteers from any and all claims, liabilities, or damages arising out of or in connection with the Participant's involvement in the Program, including but not limited to physical injury, illness , emotional distress, or property damage, whether caused by negligence or otherwise, to the fullest extent permitted by law.
3. Medical Authorization : The Parent/Guardian hereby grants permission for the Academy and its staff to administer basic first aid to the Participant and, in the event of a more serious injury or illness, to seek appropriate medical treatment for the Participant. The Parent/Guardian agrees to be financially responsible for any medical expenses incurred during the Participant's participation in the Program.
4. Assumption of Responsibility : The Parent/Guardian agrees to assume full responsibility for any and all risks of bodily injury, property damage, or any other harm that may occur to the Participant while participating in the Program, including all related events, transportation, and activities.
5. Agreement to Follow Safety Rules : The Parent/Guardian agrees that the Participant will follow all safety rules and instructions provided by the Academy staff and will act responsibly during Program activities.
